Terms of service
Last updated: 13 September 2026.
These terms set out what you can expect from us and what we ask of you when you book a session with Elevate Lifestyle Medicine. By booking, you agree to them. Please read them alongside our privacy notice and our confidentiality and safeguarding page.
1. Who we are
Elevate Lifestyle Medicine Ltd is a company registered in England and Wales, company number 17004396, registered office 54 Pembroke Road, Salisbury, SP2 9DQ. Our sessions are delivered by registered nurses. The professional title "Registered Nurse" was granted in the United Kingdom by the Nursing and Midwifery Council (NMC), which regulates our clinicians and publishes the professional rules they work to, the NMC Code. You can check their registration on the NMC register, and our NMC PINs are shown on our home page.
2. What we offer, and what we do not
We provide health education, advice and one-to-one or small group consultations about women's and girls' health. Sessions take place face-to-face in Salisbury and surrounding areas. We do not offer online consultations.
Our sessions are not a substitute for your GP, a sexual health clinic or any NHS service, and they are not an emergency service. We are both independent prescribers, but we do not issue private prescriptions through Elevate, and we do not carry out physical examinations, tests or procedures. Where we think you need something we cannot provide, we will tell you and help you get to the right service.
If you need urgent help, call 999 in an emergency or NHS 111 for urgent advice.
3. Booking a session
You can get in touch through the website, by email or by phone. We do not use booking software. We agree a date, time and venue with you by email, and your booking is confirmed once we have received your payment.
4. Fees and payment
Our fees are shown on the website and confirmed when you book. Fees include VAT where applicable. Payment is by bank transfer or PayPal. We send you the payment details by email once we have agreed your appointment, and payment is due in full before the session, no later than 48 hours before it. We do not take deposits.
Programmes running over several weeks are priced as a whole and paid in full before the first session. We do not offer instalments.
5. Changing or cancelling a booking
Life happens, so we try to be flexible. If you need to change or cancel a session:
- Let us know at least 48 hours before the session and we will rebook or refund in full.
- With less notice than that, or if you do not attend, we may charge the full fee, because the time has been set aside for you.
- If we have to cancel, we will offer another date or a full refund.
Because bookings are agreed by email, you also have a legal right to cancel within 14 days of the day your booking was confirmed, for any reason, under the Consumer Contracts Regulations 2013. To use it, tell us by email before the 14 days end. You can use the cancellation form below, but you do not have to. We will refund you within 14 days of your cancellation. If you would like a session to go ahead within those 14 days, we will ask you to confirm that request by email. If you then cancel after the session has been provided, you pay for it in full, and if you cancel part-way through, you pay a proportionate amount.
Cancellation form. You can copy this into an email to hello@elevatelifestylemedicine.co.uk: "To Elevate Lifestyle Medicine Ltd, 54 Pembroke Road, Salisbury, SP2 9DQ. I give notice that I cancel my contract for the following service: (session and date). Booked on: (date). Name: (your name). Address: (your address). Date: (today's date)."
6. Children and young people
Where a session is for someone under 18, we need a parent or carer with parental responsibility to make the booking and agree to these terms. Young people who are able to understand the session and its purpose may take part in decisions about it, including asking to speak with a nurse privately. How we handle this is explained on our confidentiality and safeguarding page.
7. What we ask of you
- Give us accurate information, and tell us if anything changes.
- Tell us about anything that would make a session difficult, such as accessibility needs, so we can plan for it.
- Treat our clinicians with courtesy. We may end a session where behaviour is abusive or threatening.
- Use the advice we give as part of your own decisions about your health. Where we suggest seeing your GP or another service, please do.
8. What you can expect from us
- Sessions delivered by registered nurses, working within the NMC Code and the limits of their training and registration.
- Advice based on recognised clinical guidance, explained clearly.
- Confidentiality, within the limits we explain before we start.
- An appropriate professional indemnity arrangement for each nurse, as the NMC requires.
9. Our responsibility to you
Nothing in these terms limits our responsibility for death or personal injury caused by our negligence, for fraud, or for anything else that cannot be limited by law. Beyond that, we are not responsible for losses that we could not reasonably have foreseen when you booked, or for any loss that arises because information you gave us was inaccurate or incomplete. Nothing in these terms excludes or limits our duty to provide our services with reasonable care and skill.
10. If something goes wrong
If you are unhappy with any part of our service, tell us as soon as you can, by email to hello@elevatelifestylemedicine.co.uk or in person. We will acknowledge your complaint within 5 working days and aim to respond fully within 20 working days. Our complaints and concerns page explains who handles it and what happens next. A concern about a nurse's professional conduct can be raised with the NMC at any time; the NMC does not deal with service complaints or refunds.
11. Using this website
The content on this website is general information about our services and about health topics. It is not advice about your individual situation and should not be relied on as such. We try to keep it accurate and up to date but cannot guarantee it. The text, logo and images belong to Elevate Lifestyle Medicine Ltd and may not be copied without permission.
12. General
These terms are governed by the law of England and Wales, and the courts of England and Wales will deal with any dispute. If any part of these terms is found to be unenforceable, the rest still applies. We may update these terms from time to time; the version in force when you book is the one that applies to that booking.
